Chronic Care Management may be available to you. - NPR

Chronic Care Management has been found to help Medicare patients reduce hospital and clinic visits.  A person who has two or more chronic conditions is eligible for a federal program that pays doctors to do more to manage the patients heath.

In spite of the availability of this program, many doctors are not enlisting in it for various reasons as explained in this article by Phil Galewitz and Holly K. Hacker published on April 17, 2024 from KFF Health News for NPR.
